Summary/Objective
The primary role of the Habilitation Specialist (HS) is to work alongside the people we support in order to help them to achieve their Personal Outcomes. This includes working with them to develop daily living, professional and informed decision making skills while also supporting their health and safety. The HS is also responsible for providing supervision and support to the Day Services Direct Support Professionals.

Responsibilities

Essential Job Functions
 Work with the people we support to help them identify and realize their personal life goals
 Advocate with and for the people we support in a variety of situations
 Work with the people we support to teach them new life skills
 Work with the people we support and other members of the interdisciplinary team to develop service plans that aid the person in achieving their personal life goals
 Assist the people we support to overcome barriers and challenges to establish and maintain a network of relationships and valued social roles
 Provide positive behavior support to the people we support as necessary
 Work in partnership with the people we support to promote their health and safety
 Prevent, recognize and report abuse toward the people we support
 Supervise, evaluate and observe assigned Direct Support Professionals
 Ensure required documentation is completed accurately and thoroughly by assigned Direct Support Professionals
 Accurately document daily service provision, monthly notes and other information as required
 Assist in training and orientation of new staff and/or floating staff
 Complete required Annual/Semi-Annual information for Individuals assigned
 Attend and present at assigned individuals’ Annual/Semi-Annual meetings

 Effectively communicate and support the organization’s mission, vision and values
 Adhere to the organization’s Code of Ethics and all other agency policies, procedures and best practices
 Always communicate in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community
 Promote and support community involvement for the people we support
 Complete all required trainings and stay in compliance
 Maintain confidentiality
 Other duties as assigned
Minimum Job Qualifications

Minimum Job Qualifications
 18 years of age or older
 High School Diploma or equivalent
 Valid driver’s license with no more than 8 points and satisfy criteria to be a driver for the organization
 Supervisory experience and experience in the field preferred
 Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential
 No convictions for crimes that preclude a person from working with members of a vulnerable population
