Responsibilities

Mobile Residential Support Team (MRST) Services Licensed Practical Nurse (LPN)

 

Under the Supervision of the RN Supervisor and Operations Director of the Mobile Residential Support Team, the Licensed Practical Nurse is responsible for the overall oversight and supervision of the healthcare needs of individuals living in Supported Housing throughout Nassau County who are at high risk for psychiatric or medical hospitalization, homelessness, incarceration or other involvement with the legal system, a lack of stability in maintaining basic health and safety needs, those with co-morbid medical or substance abuse conditions and individuals with a co-occurring intellectual disability. The LPN will work in conjunction with the MRST Administrative team to ensure the delivery of the highest quality of integrated health and continuity of care to the people we support. The qualified candidate must be able to work in partnership with all personnel within the organization to ensure the highest quality of clinical services for the people we support in a manner that is consistent and aligned with the agency mission and vision, and the philosophy of the Council on Quality and Leadership (CQL).

 

 

Qualifications

  •  

    • Completing health assessments on all referrals with identified Health and Medication related risks.
    • Collaborate with Individuals, Case Managers, Peer Support Specialist, and additional stakeholders as necessary to develop a plan to support individuals in addressing identified needs.
    • Health Service Coordination (may include coordination as well as attending some appointments if necessary in order to get appropriate care/advocate and develop care plan, especially in case of serious medical conditions).
    • Provide Health/Medical Condition education
    • Provide Medication Management education
    • Attend weekly Case/Team Meetings
    • Ability to work independently in the community
