Summary/Objective
Under the supervision of the Nursing Supervisor, the Registered Nurse (RN) is responsible for

POSITION SUMMARY
Under the supervision of the Director of Entitlements, the Collections Specialist will work in collaboration with the housing and accounting team to monitor and address program fees and rental arrears for individuals supported in FREE’s residential programs. The Collections Specialist will assist in implementing agency policies and procedures regarding rent, program fees, arrears and debt strategy.

The primary role of the Direct Support Professional (DSP) is to work alongside the people we support in order to help them to achieve their Personal Outcomes. This includes working with them to develop daily living, professional and informed decision-making skills while also supporting their health and safety.
Under the Supervision of the Assistant Director of Nursing, the Licensed Practical Nurse (LPN) is responsible for the overall oversight and supervision of the healthcare needs of the people we support in a residential setting. The LPN will work in conjunction with the Assistant Director of Nursing, Registered Nurse and the Residential Administrative team to ensure the delivery of the highest quality of integrated health and continuity of care to the people residing in our residences. The qualified candidate must be able to work in partnership with all personnel within the organization to ensure the highest quality of clinical services for the people we support in a manner that is consistent and aligned with the agency mission and vision, and the philosophy of the Council on Quality and Leadership (CQL).

Essential Job Functions
Provide for the Health and Safety of the Individuals
- Mediate emergency situations pertaining to the program according to established policies and procedures, and ensure communication of such situations in a timely fashion.
- Provide coverage until relieved in emergency situations.
Advocate for the Individuals
- Treat each individual with dignity and respect in accordance with New York State regulations and Agency policy.
- Report possible violations of individual’s rights promptly, and complete and submit incident reports in accordance with New York State regulations and Agency policy.
- Maintain the confidentiality of any information relating to the individuals and staff of the program.
- Ensure that the individual’s needs are met and intercede on their behalf as necessary.
- Participate in any investigation into incidents occurring within the program as requested and maintain confidentiality in regard to such investigations.
Admission Department Responsibilities
- Review and evaluate all applications for admissions within the organization
- Log in BH, DD and DOH applications.
- Screens all applications and determines if Family Residences & Essential Enterprises, Inc. is able to render services (county, disability, etc.) and proactively plan for future vacancy.
- Prepare BH, DD and DOH merge letters for typing
- Open a file for each application received to be screened.
- Maintain phone contact with applicant/contact person.
- Follow up on files pending information
- Arrange screening on or off site for applicant with assigned operation staff.
- Facilitate Resource & Referral meetings and present applicant profiles as necessary
- If applicant is not accepted, write letter of decline with recommendations for other resources.
- If applicant is candidate for current vacancy, organize site visit.
- Submit site visit report to Family Residences & Essential Enterprises, Inc.’s residential staff/residence for site visit.
- Follow up that site visit is complete along with assessment/paperwork in timely fashion as per regulations.
- Complete a letter of acceptance, defer or decline on applicant (including other possible resources)
- Ensure all applicants are screened within an expedited time frame.
- Ensure each applicant receives a fair, non-judgmental interview.
- Generate good inter-agency relationships.
- Notify Divisional Director of updates, concerns, upcoming vacancies, etc.
- Establish and maintain appropriate correspondence with referring agents and document all necessary information.
- Maintain a phone contact log book.
- Update and maintain BH, DD and DOH site directory and wait list.
- Assist in tracking individual movement (admissions, discharges, hospitalizations). ITC form and all correspondence regarding admissions, and discharges to governmental officials and maintain data for FREE’s strategic objectives.
- Participate in committees as deemed by Family Residences & Essential Enterprises, Inc. administrative personnel.
- Implement admission and discharge policies and procedures including assistance in obtaining alternative placements for those individuals not accepted into Family Residences & Essential Enterprises, Inc. programs.
- Attend trainings, meetings, and conferences as deemed by Family Residences & Essential Enterprises, Inc. administrative personnel.
- Review wait list files ongoing and update information.
- Attend and assist in implementing internal computerized admission department system.
- Attend and complete training in accordance with New York State and Agency regulations.
- Perform additional responsibilities, as assigned.
- Arrange and facilitate semi-annual family meetings for all new families entering the FREE network
Team Participation
- Attend all relevant SEPTA meetings, Education Fairs and Expos and Family Informational Meetings.
- Accurately and positively communicate agency philosophy and administrative decisions to all staff in a timely and professional manner.
- Report ongoing problems and suggest solutions to improve working environment/conditions.
- Establish positive working relationships with operations leads and clinical regarding admission.
- Utilize information acquired through training, meetings, education, and experience to benefit individuals served.

Position Overview
Under the supervision of the Community Habilitation Supervisor, the Community Habilitation Specialist (CHS) is responsible for assisting individuals to become more independent when performing activities of daily living and community integration in order to achieve their valued outcomes. Habilitation services are designed to develop skills in order to increase an individual’s level of independence. The Habilitation services that the Community Habilitation Specialist is responsible for organizing and implementing includes but is not limited to: training, supervision, or assistance related to communication skills, interpersonal skills, socialization, mobility, community transportation/travel training skills, problem solving skills, money management, life safety skills, etc. Services are primarily community based and revolve around volunteer and recreational opportunities. The Community Habilitation Specialist is responsible for fostering and maintaining an environment that promotes the health and safety of all persons served and must abide all agency policies, procedures, mission, vision, etc.

Position Overview
Under the supervision of Department Management, the Receptionist is responsible for the front office functions of the program. The receptionist will be an extremely organized and motivated individual who is able to communicate effectively with team members, families, and individuals receiving services to ensure operations run smoothly.
Essential Job Functions
Greet visitors, individuals and staff in a friendly manner.
Ensure visitors sign in to the visitors log and announce their arrival to the appropriate department and/or team member.
Answer all incoming calls and provide support as necessary to incoming caller.
Review all incoming faxes and distribute to appropriate department/individuals.
Mediate emergency situations to the program according to established policies and procedures, and ensure communications of such situations in a timely fashion.

The Operations Director is responsible for the overall management and supervision of a group of residential facilities. The Director’s primary role is to ensure that all of the needs of the people supported in their programs are met. These needs include ensuring their rights are both promoted and respected, supporting their health and safety, and working with them to identify and realize their personal outcomes. The Operations Director is responsible for ensuring that available resources are used efficiently and effectively, that funder and FREE, Inc. policies and procedures are followed, and that established program performance standards are met or exceeded. In collaboration with division leadership, the Operations Director is responsible for establishing and maintaining high standards of performance for their teams.
Summary/Objective
This role as the Archiving Clerk is under the supervisor of the Director of Purchasing and works in unison with the department to maintain records and monitoring all required documentation for the department
Position Overview
Reporting to Clinic Management, the CASAC will be responsible for facilitating individual and group substance use therapy as well as completing intakes, assessments, session notes, treatment plans and discharges. The CASAC will be an integral part of the interdisciplinary team servicing individuals at the Article 31 Outpatient Clinic with substance use and co-occurring disorders. This position requires strong organizational skills with a focus on efficiency, time management and the ability to multitask in a fast-paced work environment. The position may require provision of services in the community and outside of normal business hours.This is currently a 2 year position, with potential to be continued based on success of the demonstration.

Under the supervision of the Residential Manager, the Assistant Manager will be responsible for the direct supervision of the Direct Support Professionals and will provide direct oversight of service planning and the provision of clinical and health care services. The Assistant Manager is responsible for the implementation of day-to-day activities and oversight of the assigned residence.
