Summary/Objective
The primary role of the Day Services Program Without Walls Assistant Manager is to ensure that all of the needs of the people supported in the program are met while working in partnership with them to achieve their Personal Outcomes. This includes working with individuals within their community so they may develop daily living skills, professional and informed decision making skills, while also supporting their health and safety and ensuring their rights are both promoted and respected. In order to accomplish this, the Program Without Walls (PWW) Assistant Manager will lead and manage a team consisting of a Senior Habilitation Specialist, Habilitation Specialists as well as Direct Support Professionals. The PWW Assistant Manager responsible for overseeing a minimum of 80 cases as well as the HUB that they run out of. They are responsible for the completion and review of all Staff Action Plans ensuring they are followed and documented in an appropriate manner. The PWW Assistant Manager is responsible for attending all Life Plan Meetings as well as any Interdisciplinary Meetings. This position will work within conjunction with residential and clinical providers to work alongside the people they support to develop plans to meet the individual’s needs.
The Community Support Specialist provides training in independent living skills, including how to be responsible tenants and maintain psychiatric stability. This individual will assist the Program Supervisor in implementing agency policies and procedures.

Under the supervision of the Vice President of Behavior Support and Psychology Services, the Behavior Intervention Specialist (BIS) is responsible for the development, delivery, and evaluation of all behavior support services for the people they support. This includes, but is not limited to: psychological evaluations, functional assessment, behavior support programs and strategies, crisis intervention, individual and group counseling, case reviews and other interdisciplinary treatment team meetings, training and consultation with other team members. The BIS will work in collaboration with the Person and their circle of support to ensure the highest quality of clinical services in a manner that is consistent and aligned with agency mission and vision, and the philosophy of the Council on Quality and Leadership (CQL).

Position Overview
Reporting to Clinic Management, the Certified Recovery Peer Advocate (CPRA) works within Success Community Care Clinic. The CPRA will provide face-to-face services to assist connecting individuals to community based recovery supports consistent with their treatment plan. The CPRA is responsible for mentoring individuals diagnosed with mental health, substance abuse, and co- occurring disorders to develop personal and meaningful independent living goals that will help them in their process of recovery. This position requires outstanding organization and communication skills. The Peer Specialist will have lived experience utilizing mental health and/or substance abuse services.

Under the supervision of the Community Habilitation Supervisor, the Community Habilitation Specialist (CHS) is responsible for assisting individuals to become more independent when performing activities of daily living and community integration in order to achieve their valued outcomes. Habilitation services are designed to develop skills in order to increase an individual’s level of independence. The Habilitation services that the Community Habilitation Specialist is responsible for organizing and implementing includes but is not limited to: training, supervision, or assistance related to communication skills, interpersonal skills, socialization, mobility, community transportation/travel training skills, problem solving skills, money management, life safety skills, etc. Services are primarily community based and revolve around volunteer and recreational opportunities. The primary role of the Senior Habilitation Specialist (Sr. HS) is to provide direct instruction to the Community Based Pre-Vocational program, Custodial and Clerical Vocational Training programs. The Sr. HS will work alongside the people we support in order to help them achieve their Personal Outcomes as well as discover and achieve professional goals. This includes adhering to a provided curriculum to teach, evaluate and observe progress, provide outreach and coordination of volunteer opportunities for participants, supporting their health and safety, implementing all aspects of the activity schedule, participating in the annual and semi-annual review process, development and implementation of individual’s goals/services, and advocating for the needs of individuals. The Sr. HS is also responsible for providing supervision, assessment of individuals, and support to the Vocational Habilitation Specialist(s) (HS).
