Position Overview
Under the supervision of the Program Supervisor, the OMH Licensed Clinician is responsible for the
oversight of those individuals diagnosed with mental illness receiving services from FREE. This qualified,
specially trained team member will be available to provide crisis intervention, direct support,
medication monitoring and oversight, assistance with daily living skills, and any other assistance that
these individuals need while participating in site-based services. This team member will assist in the
development of the Initial Plans, Relapse Prevention Plans, and Support Plans. The Licensed Clinician will
also run therapy groups on specific topics, provide individualized supportive counseling, and conduct
assessments as needed or required. The Licensed Clinician may also be asked to undertake projects
aimed at expanding and improving the scope and quality of services provided.
Essential Job Functions
- Perform assessments to evaluate a variety of mental health and behavioral conditions to children, adolescents and adults.
- Develop an individualized, prescriptive treatment plans to fit the needs of each patient using appropriate diagnostic and patient placement criteria.
- Conduct individual, family, and group counseling, utilizing methods consistent with evidence-based practice, using a brief, focal model of care when appropriate.
- Utilize outcome tools to determine patient progress.
- Coordinate patient care with primary care, other specialty providers, schools, and families via written and verbal communication.
- Arrange for appropriate care within the FREE umbrella or through outside providers as prescribed in policies and procedures.
- Ensure that each patient’s rights are protected and that they are provided with appropriate interventions based on ability to participate, cultural concerns, and diagnostic impression.
- Participate in Departmental activities such as clinical care conferences, integrated care committees, risk meetings to ensure the highest quality of care is met.
- Maintain progress notes and other documentation required so that the client medical record is completed contemporaneously.
- Ensure that all required documentation is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.

Clinician is responsible for assisting in the development of the Support Plans, and Relapse Prevention
Plans. This qualified, specially trained team member will be available to provide crisis intervention,
direct support, medication monitoring and oversight, assistance with daily living skills, and any other
assistance that these individuals need while participating in MRST services. The Licensed Clinician will
provide individualized supportive counseling, and conduct assessments as needed or required. The
Licensed Clinician shall also be asked to undertake projects aimed at expanding and improving the scope
and quality of services provided by the Mobile Residential Support Team.

Summary/Objective
Overseen by Clinic Management, the licensed therapist (bilingual), is responsible for providing ongoing therapy, assessment and case management services to individuals admitted to the clinic. The licensed therapist is a motivated and organized individual who can manage the varying needs of his/her caseload. The licensed therapist will ensure a therapeutic and supportive milieu with the individuals on their caseload.
