Under the Supervision of the Assistant Vice President of Behavior Support Services, the Assistant Director of Behavior Support is responsible for the overall oversight and supervision of the Behavior Intervention Specialists and Behavior Support Specialists. The Assistant Director of Behavior Support is responsible for the assessment of the needs of each person receiving support as well as the development, implementation, and monitoring of behavior intervention programs. The Assistant Director of Behavior Support will provide support and coverage to identified programs within the organization as needed and is on call 24/7 for clinical assessment and guidance. The qualified candidate must be able to work in partnership with all levels of administration within the organization to ensure the highest quality of clinical services for the people we support, in a manner that is consistent and aligned with the agency mission and vision, and the philosophy of the Council on Quality and Leadership (CQL).
Overseen by the Clinic Program Leadership. The Prescriber (PA/NP) is responsible to evaluate the medical needs of individuals diagnosed with a mental health condition and who are living in temporary and/or crisis housing in Suffolk County. . The Prescriber will provide (as necessary) and review health assessments and evaluations, basic health screenings for hypertension, diabetes and cholesterol, nutritional counseling, prevention services, drug screenings and brief counseling as well as referrals to community based health care providers, as appropriate. The position requires significant travel within Suffolk County as the services are community based and has some on-call responsibilities in case of emergent situations.
Under the supervision of program management, the Licensed Clinician is responsible for assisting in the development of the Support Plans, and Relapse Prevention Plans. This qualified, specially trained team member will be available to provide crisis intervention, direct support, medication monitoring and oversight, assistance with daily living skills, and any other assistance that these individuals need while participating in services. The Licensed Clinician will provide individualized supportive counseling, and conduct assessments as needed or required. The Licensed Clinician shall also be asked to undertake projects aimed at expanding and improving the scope and quality of services provided by the treatment team. This is a Fee For Service/Part-Time position.
