POSITION SUMMARY
The Community Support Specialist (CSS) provides training in independent living skills, including how to be responsible tenants and maintain psychiatric stability. The CSS assists the Program Supervisor in implementing agency policies and procedures.

Position Overview:
NYSTART (Systemic, Therapeutic, Assessment, Resources and Treatment) works
collaboratively with the system of support to improve the lives of individuals with
intellectual/developmental disabilities and co-existing behavioral health needs/mental health
concerns. Under the supervision of the START Associate Director, the START Senior Instructor
is responsible for training coordinators to follow the START model. This includes field based
demonstration of all START based documentation, systems work, outreach, crisis planning and
other case specific trainings. The focus of the START Instructor will be to orientate Coordinators
to field based START processes as well as the orientation to START documentation.

Position Overview
Under the supervision of the Program Supervisor, the OMH Licensed Clinician is responsible for the
oversight of those individuals diagnosed with mental illness receiving services from FREE. This qualified,
specially trained team member will be available to provide crisis intervention, direct support,
medication monitoring and oversight, assistance with daily living skills, and any other assistance that
these individuals need while participating in site-based services. This team member will assist in the
development of the Initial Plans, Relapse Prevention Plans, and Support Plans. The Licensed Clinician will
also run therapy groups on specific topics, provide individualized supportive counseling, and conduct
assessments as needed or required. The Licensed Clinician may also be asked to undertake projects
aimed at expanding and improving the scope and quality of services provided.

Under the supervision of the START Team Leader, the START Clinical Linkage Coordinator is responsible for providing crisis support, linkages, outreach, and consultation services for individuals with a developmental disability and behavioral health needs.

Under the supervision of both the self-directed participant and FI Coordinator designee in a co-management manner, the Self-Direction Specialist (SDS) is responsible for the organization/implementation of activities for daily living and community integration coordinated through self-directed services. Provision of support for the person served is dependent on the individuals need(s) and may include; however is not limited to, physical lifting, pushing, transporting, moving and/or assisting the person served in the completion of job duties. The SDS is responsible for fostering and maintaining an environment that promotes the health and safety of all persons served. The SDS abides by all agency policies, procedures, mission, vision, etc., practices the code of ethics and meets the expectations of the core competencies set by OPWDD/NADSP.
